Provide supportive services at East Metro Place to give participants necessary resources and skills to achieve self-reliance.

 

  • Meet with participants regularly (75% in-home visits) to assist them in developing and achieving goals in the areas of budgeting/finances; education; employment; physical/mental/chemical health; transportation; housing; parenting and children’s goals; and to provide ongoing support through resources and encouragement.
  • Complete a Family Strengths and Needs Assessment and Goal Plan within 90 days of participant’s entry into program; update goal plan every six months thereafter
  • Provide community resources and referrals, and transportation to appointments as needed.
  • Provide 24 hour on-call service through staff pager (rotates approximately once every 6 weeks).
  • Assist Program Director with implementing onsite participant workshops (i.e. budgeting, tenant training, DBT skills training groups).
  • Assist in providing social, recreational and educational activities for residents.
  • Understanding and commitment to trauma informed care
  • Understanding and commitment to Housing first

 

Coordinate and document all work done on behalf of Solid Ground participants to maintain accurate progress and recordkeeping for participants, agency, and funders.

 

  • Maintain timely and accurate participant files.
  • Complete all forms required for the Homeless Management Information System (HMIS) within 14 days of participant’s entry into program, and update thereafter as needed.
  • Compile statistics needed for reports to funders and agency.
  • Manage time effectively to ensure quality service to participants.

 

Qualifications:

 

·         Licensed social worker (LSW) with at least three years of experience in a social service advocacy position working with clients in shelter or housing; or a combination of education and experience providing equivalent knowledge.  MSW/LISW/LICSW preferred.

 

·         Must have a valid driver’s license, reliable transportation, adequate insurance and satisfactory driving record, to transport participants as needed.

 

·         Knowledge of, and Skills and Abilities to serve families experiencing homelessness and their needs.

 

·         Ability to manage multiple priorities to ensure work is completed in timely and productive manner.

 

·         Ability to model behavior consistent with the agency mission.

 

·         Ability to travel to various locations for day and evening meetings and events.

 

·         Knowledge of community resources or ability to learn area resources.

 

·         Skill in crisis intervention.

 

·         Ability to work independently with little supervision.

 

·         Ability to carry emergency pager and be available for interventions as needed during non-office hours.

 

·         Understanding and commitment to trauma informed care

 

·         Understanding and commitment to Housing first
